A Change in booking

Hello Guys, I have recently updated the booking form for when appointments are made online.

You will notice some new fields that are required to proceed with booking including questions regarding allergies, medication, pre-existing conditions and your address - all information is completely confidential but enables me to have a clear insight into my clients prior to arrival to help me with my treatment plans.
